Unlock difficulty levels
Hard Mode: Beat the boss on level 5-6 (Cleavage) in Normal Mode on Normal difficulty
Insane Mode: Finish all of Bonus Mode 2
Nightmare Difficulty: Finish Challenge mode stage 99.
