As you have no doubt heard, in addition to retaining all the features of the 360 game (co-op, multiplayer, versus, etc.) the PC port includes new single-player content; five additional chapters including a battle with the fearsome Brumak.
The multiplayer game is expanded with three new maps and you now have access to the Unreal Map Editor - the kind of stuff Epic fans have come to expect from the company.
Multiplayer is played over Games for Windows Live, but the system had some initial problems that needed ironing out - a problem with the game update which can be solved by following the instructions on this page.
In closing, I have to say I quite enjoyed replaying Gears of War on the PC. The performance is excellent, the crispy-clean 1600x1200 resolution certainly helped and the controls feel a lot more natural on the PC. (I know, an unusual thing to say about a port, but there you have it.)
For the people who have already finished the 360 version, however, I don't quite think the added content should be reason enough to buy the game, but, certainly, if you are yet to play Gears of War, you can't go wrong by picking up the PC game. In fact, I recommend it over the 360 version.
